I think this is a great piece, I can start by saying that the highlights and shadows are placed where they should be. The band-aids covering his mouth are a great addition. As a composition, the little gestures of how his head is slightly tilted and even the way his collar folds helps break this symmetry enough that it isn't a boring or dull piece. I'm by far not an expert on this subject, but I believe you could put some contrast below his left eyebrow. If you notice the shadows under his right eyebrow there is a great sense of that depth that the skull and eyebrow help create around the eye socket. Maybe his left eye could use a little more of that? I am aware of where the light source would be (left side of his face). Last, I would say add something to his suit, it does get a little lost with the rest of the awesomeness.
